This incident report is to be completed by conduct staff, namely professional and paraprofessional residence life staff and dean of students staff, for student handbook/policy related incidents/concerns.

This safety report is be completed by campus safety staff.

Student of Concern reports can be completed by any member of the Trinity community or parents, family, and friends who are concerned about a student. Trinity’s Care Team will respond to all reports submitted on behalf of our students. Student of Concern reports are not meant for emergency situations. Please call Campus Safety and/or 911 if you are concerned about the immediate health and well-being of a student.
